Driver's License Medical DL9 Form

Book an apppointment for a Driver's License Medical assessment to complete the DL9 form for the NZTA (e.g P endorsement for Uber driving)

Why may you require a Driving License Medical Certificate

  • Passenger Endorsement: for example to be an Uber Driver

  • All people applying for or renewing a heavy vehicle licence.

  • Those who require a licence with endorsements for passengers: recovery services, driving instructors and testing officers.

  • Older drivers aged 75 or over.

  • Applicants with a medical condition that could have an effect on their ability to drive safely.

  • A medical certificate for a drivers licence may be requested at the discretion of the NZTA.

You may need to provide  a medical certificate if you suffer from any of the following conditions:

  • Diabetes

  • High blood pressure

  • Cognitive impairment

  • Joint or limb problems

  • Strokes

  • Anxiety /mental health concerns

  • Seizures, fits, convulsions, epilepsy

  • Cerebral vascular disease

  • Serious injuries (head injury)

  • Visual disturbances

  • Any health concerns which might impede your ability to drive, including fatigue, mental disabilities and dementia.

Please note that the assessment outcome may result in the determination that you are unfit to drive or that further evaluation by a specialist may be necessary
